Position Summary : Community Outreach Coordinator is s responsible for connecting Tidelands Health with the Horry and Georgetown communities. This position is responsible for finding potential partnerships or methods for engagement with the local community, maintaining a calendar for outreach events and working closely with community partners to plan and execute various outreach events. This is achieved in Tidelands Health though coalitions, grant projects, community events, and education sessions/speaking engagements.

Community Outreach Coordinator will be responsible for programming in their particular focus area but will all adhere to these overall responsibilities.

  • Responsible for engaging, finding new, and maintaining community partnerships to achieve program and departmental goals and executing outreach events together.

  • Responsible for providing community outreach events and activities to the community through our partnerships, for example: YMCA, LWGC partners, etc.

  • Responsible for teaching and leading assigned community classes including but not limited to: HeartSaver, SafeSitter, DPP, Triple P, etc.

  • Responsible for working with the clinical outreach RN to provide evidence based and approved health education curriculum to the community at churches, schools, agencies, etc.

  • Responsible for participating and leading (when needed) community coalitions relevant to the area of focus or as assigned for the department.

  • This position requires flexibility and openness to changing priorities dependent on the season for outreach (flu shots, skin screenings, etc.) and current CHNA goals of the hospital system.

  • Completion of ongoing data collection to support the program/grant goals, CHNA, and CBR.

  • Provide manager with summaries and stories of ongoing community outreach activities for the website, social media, newsletters, etc.

  • Ensuring compliance with hospital and departmental policies

  • Position provides cross-coverage and assistance within the entire unit of community outreach unit and should be able to provide coverage within unit and department as needed.

QUALIFICATIONS

Experience :

  • Three (3) or more years of experience in community outreach, community development/organizing, health education, social work, or nonprofit management required.

  • Previous experience in healthcare education setting and community coordination preferred.

  • Previous experience with coordination, leading, and managing community outreach and grant programming, preferred.

  • Prior experience in community health education and instruction, preferred.

  • Prior experience working with industries, preferred.

  • Minimum of 2 years of grant writing, management, and reporting, preferred

Education:

  • Bachelor degree in public health, health education, social work, community development, nonprofit management or other related field, required.

License/Certification :

  • Safe Sitter certification required within one (1) year of hire date.

  • HeartSaver instructor certification required within one (1) year of hire date.

  • Certified Diabetes Certification, may be required within one (1) year of hire date.

  • Certified DPP Instructor certification, may be required within one (1) year of hire date.

Knowledge/Skills :

  • Ability to proactively encourage community buy in related to the initiative goal(s).

  • Ability to work independently, adjust priorities often and establish rapport quickly.

  • Strong interpersonal and communication skills.

  • Build Relationships: establish and maintain positive working relationships with others both internally and externally to achieve the goals of coalitions and outreach events, groups and partners.

  • Proficient PC skills and experience of Windows and Microsoft office programs required.

Other :

• Personal transportation to conduct services and transport outreach materials and supplies to events and service location throughout the region, required.

• Valid SC driver’s license in good standing, required.

While performing the duties of this job, the employee is frequently (activity or condition exists from 1/3 to 2/3 of the time) required to stand, sit, and walk; frequently to use hands, fingers; and frequently to talk or hear.
