Position Summary : Program Coordinator – Diabetes Prevention Program is responsible for the delivery of the Diabetes Prevention Program (DPP) at Tidelands Health. This position is responsible for building relationships within the community and providers to facilitate program delivery, supervising daily operations of program, providing support and guidance to Lifestyle Coaches, establishing a class schedule, maintaining programming standards and trainings, and ensuring that program achieves quality performance outcomes, including HIPPA compliance, CDC reporting and compliance requirements, record keeping and coaching training records.

The DPP program coordinator will be responsible for the following:

  • Build and manage relationships within the THG practices and greater community to facilitate program delivery and referrals into the DPP program.

  • Provide regular updates and communication to direct supervisor and present program status as requested to higher leadership and board of trustees.

  • Facilitate a multi-disciplinary team collaboration and regular meetings for all Lifestyle coaches to review program standards, status, reports and upcoming classes to monitor for quality improvement and continued communication and education.

  • Recruit, retain, and supervise the Lifestyle Coaches in relation to DPP (this staff is managed through greater units) who will facilitate classroom delivery of TH’s Diabetes Prevention Program.

  • Coordinate training sessions for team members to enhance skills and knowledge. Facilitate professional development opportunities. Monitor the quality of support that Lifestyle Coaches provide to program participants.

  • Manage the program budget, track any financial assistance provided, work with manager on funds flow related to the program and ensure financial reports are reconciled.

  • Use reporting and quality assurance tools in data system to ensure TH is meeting performance standards.

  • Attend the first session of all classes and observe each Lifestyle Coach at least twice annually (during their initial 16 weekly session delivery); ensure compliance with program standards.

  • Recruit, screen, and enroll eligible participants into the program and into the data system.

  • Organize master schedule of classes offered throughout the association and assign coaches to classes in data system.

  • Support the documentation of program data within data system ensuring Lifestyle Coaches are meeting data entry deadlines.

  • Engage marketing staff to ensure adequate publicity and marketing of the program.

  • Assist Lifestyle Coaches with launch of each group and evaluation of the group.

  • Provide support to Lifestyle Coaches in using data system.

  • Assist with retention and commitment of program participants.

  • Participate in various technical assistance opportunities to share findings and best practices and to connect with other hospitals and community partners participating in the program.

  • Provide class coverage if needed to prevent a cancelled class.

QUALIFICATIONS

Experience :

  • Experience in program coordination/management required

  • Education in or experience with health promotion programs and nutrition (preferred: Diabetes Prevention Program)

  • Previous experience in healthcare education setting and community coordination preferred.

Education:

  • Bachelor degree in public health, health education, social work, community development, nonprofit management or other related field, required.

License/Certification :

  • Certified DPP Instructor certification, required within one (1) year of hire date.

  • Certified Diabetes Certification, may be required within one (1) year of hire date.

Knowledge/Skills :

  • Ability to work independently, adjust priorities often and establish rapport quickly

  • Strong interpersonal and communication skills.

  • Proficient PC skills and experience of Windows and Microsoft office programs required.

Other :

• Personal transportation to go to various DPP classes in service area and promote classes

• Valid SC driver’s license in good standing, required.

While performing the duties of this job, the employee is frequently (activity or condition exists from 1/3 to 2/3 of the time) required to stand, sit, and walk; frequently to use hands, fingers; and frequently to talk or hear. The employee must exert 15 to 20 pounds of force occasionally (activity or condition exists up to 1/3 of the time), and/or 5 to 10 pounds of force frequently, and/or greater than negligible up to 5 pounds of force constantly to move objects.
